Student Success Card 

With a focus on supporting safety and academics, Middletown City Schools will issue Student Success Cards to all students K-12, beginning in August 2025.

The purpose of these cards is to increase efficiency, and most importantly, enhance student safety.  Student Success Cards are IDs with the student’s name, ID number, and logo. PreK-6 students will not have their photo on their card. Students in grades 7-12 will include their photo.

During the school year, these Student Success Cards can be used to: 

  • Serve as ID during the school day and athletic events
  • Access the school bus 
  • Check into school when arriving late and out of school when leaving early 
  • Provide another way to access school lunch account 
  • Additional services such as checking out books from the library will be added in the future 

Students are expected to receive cards and lanyards by mid-August and must present them when boarding and un-boarding the bus.  Junior High School and High School Students must carry them all day.  

Students riding the bus must carry their Student Success Cards with them every day.  

Frequently Asked Questions

How do I get a new or replacement card?

If you have lost or damaged your card, a new card can be requested by notifying the secretary at your student’s school. If your card was recently lost, we ask that you look for one day before reporting the card as lost. If it is still lost after that time, please notify the school secretary and a new card will be issued.

The first card and one replacement card are provided free of charge.  There is a $2 dollar fee for the second replacement card and any additional cards, thereafter.  The fee will be added to the students ProgressBook account as replacement cards are issued.

What if my student’s card has the wrong or old photo on it?

If your student’s card has an incorrect or outdated photo, please notify the school secretary.  Once a new photo has been taken and uploaded to the student information system, the school secretary will be able to request a new card.

What if I move and my student changes schools?

Students moving within the school district will keep the same card to be used at the new school.

Will this card be needed to get to lunch or for student activities?

PreK through third grade students or those that do not know their PIN may use their Student Success Card in order to get breakfast and/or lunch.  Students may also use the card to check into school when arriving late or check out of school when leaving early.

Does my student have to wear the lanyard and card?

Yes, for students 7 -12. Students are expected to wear the Student Success Card both on the bus and during the school day.  All others must present the card for boarding and un-boarding the bus.  It is recommended that the Student Success Card be attached to the students’ bookbag for bus riders.  Individual elementary schools may have additional guidelines for use and carrying them during the day.
