Current Student Transcripts

To request records for Students transferring to another school, please email a records request with a parent/guardian signature to: MHSRECREQ@middletowncityschools.com

Students must request their transcript to be sent to Colleges/universities from Schoolinks. SchooLinks is accessed via Clever. If you have any questions, Email Mindy Clark at mdclark@middletowncityschools.com

Former Students and Graduate Process:

Middletown High School has partnered with Parchment.com to provide secure online transcript ordering available 24/7. Creating an account is free and only takes a few steps. Please note that all transcript requests through Parchment have a processing fee.

Graduates Before 2008:

To order a transcript if you graduated before 2008 there is a $5.00 fee. The Transcript Request Form is below with instructions.
